This policy is applied to all members of the Oz Locums Pty Ltd team.

CONSENT

By submitting your personal information to Oz Locums Pty Ltd, you consent to Oz Locums Pty Ltd dealing with your personal information in accordance with this Policy. In addition, if applicable law requires your specific consent to deal with particular personal information, you will be asked to give that consent. Your personal information will be used, stored, disclosed and treated according to this Policy.

HOW WILL THE DATA BE STORED?

Your information is stored in our database and cloud storage. This database is operated on a server that allows disclosure to cross border recipients only as required for the performance of our services. The database has restricted user access.

HOW WILL THE DATA BE SHARED?

Data will be shared after express consent is given by the candidate, with potential employers in Australia. It will be shared verbally with prospective employers and electronically over the internet.

HOW LONG WILL THE DATA BE STORED?

For the purpose of sourcing short and medium term employment with Oz Locums Pty Ltd we have a promise to retain personal information for five years since the last communication. Should Oz Locums Pty Ltd not have any communication either verbal or written over a five year period of time Oz Locums Pty Ltd will permanently delete personal records from its systems.

HOW DO WE MAINTAIN AND KEEP RECORDS OF DATA SHARING?

• Results of inquiries that Oz Locums Pty Ltd make of former employers, work colleagues, professional associations or registration body;

• Results of any competency or medical tests;

• Performance feedback (whether positive or negative);

• Additional information which the Medical Officer has provided to Oz Locums Pty Ltd.

Personal information may be disclosed to:

• Potential and actual employers and clients of Oz Locums Pty Ltd;

• Other consultants at Oz Locums Pty Ltd in connection with possible employment opportunities;

• Any person or body with a lawful entitlement to obtain information.

If the Candidate does not provide Oz Locums Pty Ltd with the information sought, Oz Locums Pty Ltd may be limited in our ability to locate suitable employment. This is explained in the Legitimate Interests Assessment.

CONTACTING CANDIDATES

Oz Locums Pty Ltd may contact subscribed Candidates by the email, telephone or written document to:

• Give information on the progress of an application;

• Respond to a request for information or a product or service;

• Seek permission to pass on the resume/CV for a potential placement or employment opportunity;

• Provide Candidates with updated information about our website, and

• Provide Candidates with information about our products and services which they may find useful.

IF A CANDIDATE WANTS TO WITHDRAW CONSENT FOR CONTACT FROM OZ LOCUMS PTY LTD:

A Candidate may remove their name from the Oz Locums Pty Ltd list by emailing Oz Locums Pty Ltd at info@ozlocums.com and asking to be removed from the mailing list.

SUBJECT ACCESS REQUEST

• Oz Locums Pty Ltd will always provide a copy of all personal information held, should it be requested by the Candidate within one month of the request being made (or within two months if the request is particularly complex) at no charge.

• Oz Locums Pty Ltd will correct data if required as soon as practically possible, at the request of the individual concerned at no charge.

DATA PORTABILITY AND DELETION

• Oz Locums Pty Ltd will honour the right of a data subject (Candidate) to receive the personal data concerning them in a ‘commonly use and machine-readable format’ and have the right to transmit the data to another "controller".

• Oz Locums Pty Ltd will uphold the right to be forgotten when requests are made to delete all data held on them from our databases.

• This will be done when request is made via our website, by telephone, email or written request.
